School Information
Sixth Grade Students Who Have Previously Attended Marshall County Schools Must Have:
- A school physical received within the past 12 months
- A second (2) measles, mumps, and rubella (MMR) vaccine received after age 4.
- Hepatitis B vaccination series prior to sixth grade entry.
- An appointment time is not acceptable. Immunizations must have been received before the first day of school.
Transfer Students From Other Kentucky Counties or States Must Have:
- A physical exam within 12 months prior to initial admission to a Kentucky school.
- All immunizations (with all previous dates) must be up to date and recorded on a Kentucky immunization certificate. You may check with the local health department (Marshall County Health Department, phone 270-527-1496) or school nurses to have immunizations placed on a Kentucky certificate.
- Vision exams by an ophthalmologist preferably recorded on a Kentucky Eye Exam Form.
Middle School and High School Athletes Must Have:
- Any student that plans to participate in school-sponsored sports must have a sports physical. A physician must must do these physicals. (ARNP, PA or health department physical is not acceptable. [This is according to KHSAA regulations])
- Sports physicals are offered free of charge to athletes during the summer months. Notices will be given at school as to the dates of the physicals. If you do not wish to receive the physical at school, the physical will need to be obtained at your own expense.
- Sports physicals are valid for a period of one year or 12 months. Another physical must be obtained to participate in sports once a physical has expired.
